Transition Support

For any Child or Young Person, transition points in their life can cause difficulties (e.g. School to work or further training, leaving home).

For a Child or Young Person who has Special Needs or has a Disability, this transition can present a significant challenge, especially if that person is alsohaving to change the way they are supported.

For this reason, Children’s Services has a Transitions Quality Development Officer, who will support and guide people and their carers through this transition to ensure that the services they currently engage with are more ‘joined up’ and planned during transition.

Their primary role is to ensure that the Transition Plan for Children and Young People who are currently supported through Children’s Services and are in ‘transition’ to Adult Services, are supported to ensure that this process is as seamless as possible, and that there are no surprises and that their needs are being met.
