Health and Safety Online PO3

All incidents that result in a personal injury, damage to property or a near miss/hit must be reported immediately to the Officer in Charge/Head of Department at the time of occurrence and made the subject of an Accident report (PO3).

RTC’s are exempt - however, collisions that result in damage to property/equipment on Service property are not. This will require the completion of a form PO3 as soon as is practicably possible.

The person completing the PO3 cannot be the same as the person involved in the accident/incident. This is a requirement of the County Council to comply with Health and Safety Regulations. Someone else must complete the PO3.

Report an accident

From 19 June 2018 you will no longer be able to access the current on-line PO3 Incident reporting system. Due to unforeseen circumstances the existing contract has been terminated early and the new system isn’t yet ready for use. During this period can you please download the hard copy version of the PO3 form for completion instead, and email it to us.

Please email the completed PO3 form to corporatehealth&, ensuring that in the subject heading of the email you include:

- Employee (Empl)or non-employee (non-empl)
- Date of the incident
- Directorate

Example of email subject heading: “empl/01.01.18/CS” (i.e. CS is abbreviation of Children’s Services which is relevant to schools). Updates on the progress of the installation of the new PO3 system will be made on this page and via Perspective Lite.
