Health and Safety Online PO3

All incidents that result in a personal injury, damage to property or a near miss/hit must be reported immediately to the Officer in Charge/Head of Department at the time of occurrence and made the subject of an Accident report (PO3).

RTC’s are exempt - however, collisions that result in damage to property/equipment on Service property are not. This will require the completion of a form PO3 as soon as is practicably possible.

The person completing the PO3 cannot be the same as the person involved in the accident/incident. This is a requirement of the County Council to comply with Health and Safety Regulations. Someone else must complete the PO3.
